Phyllis Darlene Chalack
1943 - 2009
Phyllis Chalack was born on April 9, 1943 in Wetaskiwin, Alberta and passed away on August 12, 2009 after a courageous battle with ALS (Lou Gehrig’s Disease). She will be sadly missed by her beloved husband of 43 years, Neil; her three sons and their spouses: Kelly and Analia, Cory and Brandi and Jeff and Tina; two grandchildren Ryley and Cianna and her many close and dear friends. Phyllis is also survived by her sisters Lorraine (Dale), Betty (Gordon) and Donna; sister-in-law Bernice and their families. Phyllis, your love, laughter and enjoyment for life will be truly missed but you will always remain in our thoughts and hearts forever. We love you. A Funeral Service was held at the Trinity Lutheran Church on Tuesday, August 18, 2009 at 2:00 p.m. with Reverend David Schumacher officiating. Interment followed in the Forest Home Cemetery. In lieu of flowers, donations may be made to the ALS Society.
